Eligible Appliances: Refrigerators, freezers and clothes washers
Rebate Program Details:
- Customers can receive a cash rebate of up to $105 by purchasing a new ENERGY STAR® qualified refrigerator, freezer or clothes washer starting February 12th, 2010, and while funding remains.
- The customer's new ENERGY STAR qualified refrigerator, freezer or clothes washer purchase must replace their existing appliance of the same type in their home.
- If customers recycle their old, existing appliance of the same type, they will receive the maximum rebate amount.

Rebate Program Details:
- Customers can receive a cash rebate of up to $555 by purchasing a bundle of three (3) High Efficiency ENERGY STAR appliances
(a combination of a refrigerator, clothes washer and a dishwasher - maximum 1 of each type) starting February 12, 2010, and while funding remains.
- The customer's new High Efficiency ENERGY STAR refrigerator, clothes washer and dishwasher purchase must replace their existing appliances of the same type in their home.
- If customers also recycle their old, existing appliances of the same type, they will receive the maximum rebate amount of $555.
- The refrigerator, clothes washer and dishwasher must meet the Consortium for Energy Efficiency (CEE) guidelines: a) Refrigerator must be from CEE Tier 2 or 3; b) Clothes Washer must be from CEE Tier 2 or 3; and c) Dishwasher must be from CEE Tier 1 or 2.

There are two ways customers may apply for the rebate program once the ENERGY STAR appliance(s) have been purchased and, if applicable, their old appliances have been recycled:
- Fill out the Application Form online www.nyapplianceswapout.com/application.aspx and mail in the required proof of purchase and/or proof of recycling information.
- Or call the program hotline at 1-877-NYSMART to complete the form over the phone, then mail in the required proof of purchase and, if applicable, proof of recycling.
Helpful Retailer Tip:If you have the capability, you may wish to set up a computer kiosk in-store where your customers can apply for the rebate on the spot.

Note on Option 1 Single Appliance Rebate: Only the
original new appliance purchase receipt will be accepted for the rebate. Copies are acceptable only when multiple applications are submitted for purchases on the same receipt.
Note on Option 2 Appliances Bundle Rebate:Only the
original new appliance purchase receipt(s) will be accepted for the rebate. Photocopies of receipts will not be accepted.
1. Appliance type (refrigerator, freezer, etc.); 2. Make; 3. Model; 4. Name of company/location where recycled; 5. Date of recycling pick-up of old appliance; and 6) Approximate age of recycled appliance.
Recycling receipts are required for your customers to receive the maximum rebate amount. If you do not currently provide a recycling receipt and would like to offer this service to you customers, we will provide a program approved receipt at your request. Please contact our hotline at 1-877-NY-SMART or via email at info@nyapplianceswapout.com to request a receipt. The receipt provided is assigned a specific retailer number and is only intended for your sole use and should not be shared with anyone else.

- Recycled appliance type must match purchased appliance type.
- In order to receive the maximum rebate amount offered, the customer must recycle the replaced appliance (see tables on previous page). A receipt providing proof of recycling, including make and model of the replaced appliance, must be submitted with the completed application. For New York City residents, confirmation numbers from the NYC 311 hotline will also be accepted as recycling verification for refrigerators and freezers. If NYC residents wish to receive a rebate for clothes washers and dishwashers, however, Department of Sanitation (DOS) does not provide a confirmation number for recycling them, so residents must have a store recycle them in order to receive the maximum rebate for recycling.
If you do not provide an appliance recycling service, let your customers know there are many local recycling centers and transfer stations they can contact to properly recycle their old appliance(s). A list of local recycling centers can be viewed online at

http://www.dec.ny.gov/docs/materials_minerals_pdf/tslistregist.pdf.
Applications will be accepted on a first come, first served basis, and rebates will be provided based on the availability of program funding, which may be exhausted at any time without notice. However, as soon as the customer fills out the application online (or calls to have the application filled out) and receives a unique Rebate Reservation Number, the rebate money is reserved for that customer. If an application is incomplete, the participant will be notified of corrective action required and rebate money will remain in the funding pool pending submission of a corrected application.
If the participant does not submit a timely application or submits an application that fails to qualify (does not meet all rebate criteria), the application will be denied.
